Modern science has shown that the widely held beliefs of clinicians about urinary tract infection (UTI) are wrong. A large body of meticulous, rigorous data, from different centres around the world makes this point. How can it be that doctors continue to practise in contradiction of what we now know? A few clinicians are now changing their approach with gratifying results so it is timely to encourage others to do likewise. Clinical guidelines have achieved such influence that most doctors feel compelled to follow them and may face censure if they do not. Regrettably the guidelines are mistaken and contradict the known science. The inertia of bureaucracy and the fear of antimicrobial resistance (AMR) do not help to encourage reflection. However, things are changing and the future should see new and better informed advice. It is a tragedy that these circumstances are leading to widespread suffering amongst many women, some men and children who experience untreated or inadequately treated infection that may plague them for years. This situation has to change. This book sets out the truth about this neglected field and explains the many errors that haunt the topic. The style makes the message accessible to all clinicians. The story is convincing, because the clinical stories that illustrate the text will be so familiar to practising clinicians, who have been baffled by their experiences. Above all, this book will help you and your patients by detailing an accessible, practical approach to resolving this difficult clinical problem in common practice. The scope of the book will cover: the history of the medicine of urinary tract infection (UTI); the urinary microbiome and what the microbes are really up to; the battles between the pathogens and the innate immune system; the truth about the tests and the criteria used to define UTI; antimicrobial resistance and the importance of Darwinian evolution; the science and ground-breaking research on UTIs; the use of antibiotics; successful treatment; supportive and other related treatments; ethics; the future; and, above all, the experiences of the patients. James Malone-Lee is an Emeritus Professor of Medicine at University College London (UCL). For 37 years as a clinical scientist at UCL he studied lower urinary tract symptoms. His research group made discoveries that challenged numerous strongly held beliefs about lower urinary tract disease, particularly infection. For many years this new knowledge was rejected by many, but in the wake of corroborative evidence from others around the world, this new thinking is becoming more widely accepted.

'Crucial reading for us all' Stylist 'Remarkable, impressive and important' Maisie Hill, author of Period Power 'Validates for women that their experiences are not all in their heads' Dr Sarah E. Hill, author of How the Pill Changes Everything An inclusive and empowering manifesto for change in women's healthcare – exploring the systemic and deep rooted sexism within medicine, and offering actionable ways for women to advocate for ourselves and others and get the diagnosis and treatment we need. Have you ever been to a doctor and felt like you were being fobbed off or ignored? Did they belittle or overlook your concerns about your health? Ever been told you're just 'hormonal'? You're not alone. Women make up 51 per cent of the population and are the biggest users of healthcare services – for themselves and as mothers and carers. But all the research shows there are massive gender differences in men and women's healthcare. Our pain and suffering has been disbelieved; we are misdiagnosed, given tranquilisers when we need painkillers, antidepressants when we need HRT, and not trusted to make informed choices about our own bodies. As women speak out about their experiences of gaslighting and misdiagnosis, health journalist Sarah Graham investigates what it will take to bridge the gender health gap. Meet the patients, doctors and campaigners who are standing up and fighting back, and find practical tips on advocating for your own health. Be inspired by stories that will incite and offer hope. You're not alone, you're not going mad, and we believe you.

The key to feeling good “down there” is understanding causes and solutions—there is relief from pelvic pain. One in fifteen people report experiencing chronic pelvic pain, while many more suffer in silence. It's time to put aside the stigmatization and misdiagnosis! Dr. Sonia Bahlani has helped thousands of patients reclaim their lives at her New York City practice. With dual expertise in the fields of gynecology and urology—sought purposefully because she was interested in helping patients with pelvic pain—Bahlani is the doctor to address this common yet often-untreated condition. In this book, Bahlani helps readers in multiple ways: she provides detailed information on what might be causing pain—from endometriosis to fibroids to prostate inflammation—through descriptions, questions, and case studies; and she reviews treatments, from what readers can expect in a doctor’s office to what they can do at home. Her sympathetic bedside manner and extensive knowledge will help empower readers and give them renewed control over their own bodies.

Lower urinary tract symptoms may be the result of a structural or functional abn- mality. To consider either in isolation would be an incomplete assessment, but c- bined anatomical and functional imaging provides a more powerful and complete tool for investigation of lower urinary tract malfunction. Yet many of the currently available radiological texts concerning lower urinary tract imaging fail to discuss these two aspects in a holistic manner. We hope that this book will fll this gap. Although it is written to be as comprehensive as possible such that it covers the vast majority of lower urinary tract disorders that will be encountered in adult urological practice, it is not meant to replace conventional textbooks of uroradiology. Rather, it is meant to be practical and user friendly. It is intended for everyday clinical use; it should allow the trainee and trained radiologist or urologist to plan a logical, yet complete radiological and dynamic investigation of the lower urinary tract. It should also be useful for nurse practitioners, medical students, and all other medical staff who commonly manage lower urinary tract disorders.
