Initiated in 1975, the Eberbach/Wiesloch Study is a prospective preventive study modeled after the Cardiovascular Comprehensive Community Central Programme of the WHO, with the aim of lowering the incidence of cardiac infarctions in a population representative of that of the Federal Republic of Germany. Part of the study is devoted to a description of smoking habits, and in particular, of the effect of cigarette smoking on plasma lipid and lipoprotein levels. Important components of this investigation were the inclusion of plasma levels of cotinine (a stable metabolite of nicotine and the only precise parameter of cigarette consumption), as well as the correlation of cotinine with other plasma parameters. Not only are these studies unique in the Federal Republic of Germany, but they also provide significant insight into the development of arteriosclerosis in connection with the established risk factors of smoking and hyperlipidemia. The data collected point to a considerable influence of smoking behavior on plasma lipid protein parameters.

The survey of the following epidemiologic studies of "Institut fUr Arterioskle roseforschung" in MUnster was given: 1. Follow-up study to analyze longterm mortality after myocardial infarction and individual risk of death. 2. Secondary preventive study after myocardial infarction under the conception of mesenchymal suppression. 3. Prospective study to find out the predictive power of cholesterol sub fractions. 4. Study of old age probands. The advantage of our infrastructure of the epidemiologic work lies in the close connection to the clinical medicine, as the scientific planning and guidance is done by physicians and teachers of university clinic. There is less danger that irrelevant problems are followed or that non-medical researchers like statisticians will domi nate the studies. We have deliberately avoided to join or organize multicenter stu dies believing that a larger output of data cannot outweigh the disadvantages of possible mistakes and errors through lack of contact and controversial conceptions among the coworkers. We hope to show that studies planned for screening of as many as 10 000 probands can be performed at relatively low expense. An im portant point is the adaptability of our system, which enables us to enlarge the already running studies for instance by addition of parameters like blood coagula tion as is being prepared by the team of Professor van de Loo in MUnster. This monograph summarizes the results of a fifteen-year vascular pathology research project sponsored by the Hungarian Academy of Sciences. The workers who have participated in this project are indebted to the Academy for financing extensive investigations and visits to institutes abroad, the aims of which were to solve certain problems by international cooperation. The results presented by the collective effort of research teams, each studying here have been achieved a particular problem, but working together to the same end. Mutual discussion of findings and a regular exchange of information between the research groups have contributed to the progress of the investigations. Regular pUblication of details and mutual criticism ensured that the findings were uniformly interpreted. It is hoped that the fundamental aim of our long-term project, i. e. the selection of the right approach to the complex problem of vascular pathology, has been achieved. The new approach results from the recognition that since arteriosclerosis finally leads to various kinds of vascular injury, attention should be focussed on the events preceding the development of the lesions rather than on the lesions themselves. This claim has been confirmed by various model experiments in which vascular injury inflicted under different conditions uniformly led to morphologi cally verified arteriosclerotic change. The recent increase in the incidence of civi lization diseases of the vascular system has resulted in a world-wide expansion of arteriosclerosis research.
